Extremely common in moments of intense anger or threat. The strength of the insult can vary with tone: it can express fury or, between very close friends, grudging respect. Context determines everything.
Always followed by a noun phrase: 'me cago en Dios', 'me cago en mi puta vida', etc. The full phrase is a fixed exclamation of extreme frustration or disbelief. The literal meaning is irrelevant; the whole structure functions as an outburst.
A fixed phrase used to dismiss someone or something with contempt, or to express that a situation has gone badly wrong. Can be directed at a person or used as a general expression of exasperation.
